Severus's first revelation is this: Harry pushed to return the Dementors to Azkaban for his sake alone. He realizes it when he sees the knifelike pleasure in Harry's eyes every time he asks "do you remember doing this last time?", and Severus answers "no." Severus remembers the visits, of course: the curdling of suppressed fear every time the door unlocks, the bitter tang of seeing Harry's face and knowing why he visits, the shameful longing and regret when Harry leaves. What he doesn't remember is what comes between, although he knows it's sex because of the cold wet mess that Harry never bothers to clean. So each time, the memories get written freshly - soft clean lips against his own, hands that touch him with gentleness and care, whispered words that promise freedom - and each time, before the last door clangs shut on Harry's departure, the Dementors glide in and steal everything but the regret.
The second revelation comes more slowly, and it brings the kind of fierce searing joy that the Dementors can't steal because it has nothing to do with hope. Severus finally realizes that Harry's falling in love with him. And this is the sort of love that Gryffindors fall into, the kind where they're too noble to do anything but hate themselves for feeling it. Harry never admits anything - he could be married with six children, for all Severus knows - but Severus realizes what's going on when Harry stops fucking him like he wants to give Severus hope and starts fucking him like he's trying to deny hope to himself. Sometime around then, Severus starts remembering the sex, and he can only conclude that the Dementors realize he'll never again hope for anything more.
Severus can't remember how many years pass before he has the third revelation, but after that one, he remembers everything. And this is the third revelation: that no matter what he does or how long he waits, Harry will never, ever forgive him.
